Transportation handled with the dignity these moments deserve.

A hospice patient’s trip home, to family, or to inpatient hospice care isn’t just a ride. It’s a moment that families remember. We treat it that way.

For the trips that aren't really about logistics

By the time a hospice patient needs transportation, the family has already navigated more than most people will in a lifetime. The right ride home from the hospital. A final visit to the family house. A transition to inpatient hospice. A gentle move from one care setting to another.

These trips don’t show up on a route optimization dashboard. They show up in family memories — for years afterward. Either as a moment of grace, or as another thing that went wrong on the worst day.

We chose to build the kind of NEMT partner you can hand a hospice patient to without worrying.

Quietly. Carefully. Without rushing.

The drivers we send to hospice trips are the ones who’ve been with us long enough to read a room. They know that the family in the lobby may have just made an unimaginably hard decision. They know that the patient in the wheelchair may be having their last conversation with a sibling.

We don’t talk about it that way to families. We just show up on time, move at the patient’s pace, take the route they want even if it’s longer, and let the moment be what it is. Then we leave them to it.

Built around how hospice teams actually work

Direct intake coordinator line

Your hospice intake or admissions team has a dedicated line to our dispatch. No portal log-in required for urgent transitions.

Family-aware communication

We send SMS updates to whichever family member you designate — quietly, factually, without intruding. Pickup confirmed. En-route. Arrived safely.

Same-day response standard

Hospice transitions rarely happen on schedule. Our partner accounts include same-day pickup with confirmed arrival windows. We’ve moved patients within two hours of a call.

Who this page is for

  • Hospice intake and admissions coordinators
  • Hospice social workers and bereavement coordinators
  • Inpatient hospice directors and DON
  • Hospital case managers placing patients into hospice
  • Senior living and memory care directors coordinating end-of-life transitions
  • Home health agencies handing off to hospice care

What we’ll always tell you straight

We’re not a hospice agency. We don’t provide medical care during transport, manage symptoms, or replace the role of your hospice nurses and aides. What we do is move patients carefully, on time, with drivers who understand the gravity of the trip — so your team can focus on the care work that only your team can do.

If a patient needs medical transport with clinical accompaniment, we’ll tell you when we’re not the right fit and help point you toward who is. Honesty matters more here than billable miles.

Set up a hospice partnership

We’d be honored to be your hospice transportation partner. Tell us about your patient population and how your team handles transitions. We’ll outline how a partnership works.
